Initial Consult:​

First, you need to book an appointment with the doctor. Afterward, a visit to a doctor for consultation and tests for the further proceeding. The success rate depends on many factors from the couple’s age to our treatment approaches. You and your partner need to go through some screenings before instigating the IVF cycle. The screening includes some medical tests like semen analysis, ovarian reserve testing, and checking on your medical history if any infectious disease like HIV, or AIDS. In addition to this, a test of the uterine is conducted too. It will make ease your treatment and the chances of pregnancy will increase.

Ovarian Stimulation​

IVF cycle begins from this stage which involves several junctures like ovarian stimulation, egg retrieval, sperm retrieval, inseminations, and embryo transfer. One IVF cycle may take 2-3 weeks approximately. The female partner is provided with the medications to stimulate ovaries for egg growth. Only some of them will get enough mature to get fertilized. Few medications may be used to put a stop to premature ovulation, for preparing the lining of your uterus for that you may be recommended by your doctor to begin taking progesterone. Some injections might be given to you containing FSH (Follicle-stimulating hormone) or LH (luteinizing hormone) or combined to stimulate egg quantity.

Ovulation induction​

In IVF hormones are injected to stimulate your ovaries to produce multiple eggs — rather than the single egg that normally develops each month. Multiple eggs are needed because few eggs might not fertilize, and few embryos may arrest or may be of poor grade

Medications given are​

• for ovarian stimulation. To stimulate your ovaries, we administer injectable hormones like FSH (follicle-stimulating hormone) and LH (luteinising hormone).
• for oocyte maturation. Once follicles are mature, trigger injection is administered like human chorionic gonadotropin (HCG) or GnRH agonist to help achieve final oocyte maturation.
• to prevent premature ovulation. GnRH antagonists like cetrorelix/ganirelix or GnRH agonists like lupride or gonapeptyl are administered to prevent premature ovulation or premature progesterone rise.
• Ovarian stimulation usually lasts for about 8-14 days.
• To determine when the eggs are ready for collection, we perform:
• Vaginal ultrasound, an imaging exam of your ovaries to monitor the development of follicles — fluid-filled sacs where eggs mature
• Blood tests, to measure your response to ovarian stimulation medications — estrogen levels typically increase as follicles develop, and progesterone levels remain low until ovulation

Egg Retrieval

Egg retrieval is done 34 to 36 hours after the final injection and before ovulation. For egg retrieval, you shall be admitted at about 8-9 am on an empty stomach. Egg retrieval is performed under general anaesthesia. Transvaginal ultrasound aspiration is the usual retrieval method. An ultrasound probe is inserted into your vagina to identify follicles. Then a thin needle is inserted into an ultrasound guide to go through the vagina and into the follicles to retrieve the eggs. The follicular fluid is aspirated through a needle connected to a suction device. Multiple eggs can be removed in about 20 minutes.

After egg retrieval, a patient may experience mild lower abdominal pain or minor vaginal bleeding. Follicular fluid is then screened for cumulus-oocyte complexes(COCs). For IVF eggs that appear healthy and mature will be mixed with sperm to attempt to create embryos. However, not all eggs may be successfully fertilized.) For ICSI, COCs are incubated for 2 hours, following which they are stripped, metaphase 2 oocytes are then injected with sperms.

Fertilization

The insemination can be carried out by various methods as per requirement. In conventional fertilization, the healthy eggs and healthy sperms are kept overnight under a controlled lab environment to form a mbryo. Whereas, in Intracytoplasmic Sperm Injection, healthy sperm is directly injected into the mature egg. They are kept under observation until it results in a healthy embryo. If the embryo is not healthy then it may lead to failure of the IVF cycle.

Embryo Transfer

Embryo transfer is performed 2 to 5 days after ovum pickup. The procedure is usually painless and doesn’t require anaesthesia in most cases. Embryos are inserted via a long, thin, flexible tube called a catheter into your vagina, through your cervix, and into your uterus. A syringe containing one or more embryos suspended in a small amount of fluid is attached to the end of the catheter. Under trans abdominal sonography guidance, once the catheter is placed in the proper position, embryos are injected. If successful, an embryo will implant in the uterus about 2 -5 days after embryo transfer.

Pregnancy Test

About 14 days after embryo transfer, we will test a sample of your blood to detect whether you’re pregnant. If you’re pregnant, we will ask you to continue the medications and schedule you for an ultrasound after 4 weeks of embryo transfer. If you’re not pregnant, you’ll stop taking progesterone and likely get your period within a week. We will perform a few tests to determine the cause of failure and apply a few more modifications to improve your chances of getting pregnant.

Sperm Retrieval​

Your partner will be asked to produce a semen sample through masturbation. Sperm will be separated from seminal plasma. In certain cases like when a partner is diagnosed with azoospermia ( zero sperm count ), sperms can be retrieved by needle aspiration from epididymis or testis. Fertilization can be attempted using two common methods: Conventional insemination: During conventional insemination, healthy sperm and mature eggs are mixed and incubated overnight. Intracytoplasmic sperm injection (ICSI): In ICSI, a single healthy sperm is injected directly into each mature egg. ICSI is often used when semen quality or number is a problem or if fertilization attempts during prior IVF cycles failed.
